Telehealth Counseling
Telehealth therapy (also called teletherapy or online therapy) delivers mental health services remotely using technology—most commonly video conferencing, phone calls, secure messaging, or specialized apps. It offers the same core therapeutic approaches (CBT, psychodynamic, EMDR-adapted methods, couples therapy, etc.) as in-person care, adapted for a virtual setting.
How it works
Platform: Sessions occur on a secure, HIPAA-compliant video platform or by phone. Clients receive a meeting link or login instructions. Some therapists also use secure texting or portals for scheduling and homework.
Scheduling: Appointments are scheduled like in-office visits. Clients join from a private, quiet place at the agreed time.
Intake & consent: Therapists complete intake paperwork and obtain informed consent that covers telehealth-specific issues (privacy, emergency procedures, technology limits).
Session flow: The therapist and client meet virtually, conduct assessment, set goals, and use therapeutic techniques. Sessions typically last 45–60 minutes.
Documentation & billing: Therapists document sessions in their clinical record and bill insurance or process payment like traditional therapy. Coverage varies by plan and state.
Benefits
Access: Removes geographic barriers—useful for people in rural areas, with mobility limits, or limited transportation.
Convenience: Reduces travel time; easier to fit therapy into busy schedules.
Continuity: Allows ongoing care during illness, travel, or public-health restrictions.
Comfort: Some clients feel safer or more open discussing issues from their own environment.
Expanded provider options: Clients can access therapists who specialize in specific issues even if those therapists are out of state (subject to licensing rules).
Limitations and considerations
Licensing & jurisdiction: Therapists must be licensed to practice in the state where the client is located at the time of the session. Cross-state care may be restricted.
Technology: Reliable internet, a device with a camera/microphone, and basic tech skills are required. Technical problems can interrupt sessions.
Privacy: Clients need a private space; shared or thin-walled environments may compromise confidentiality. Public Wi‑Fi and unsecured devices increase risk.
Clinical suitability: Telehealth is appropriate for many conditions (anxiety, depression, relationship issues) but may be less suitable for severe instability, active suicidality, uncontrolled substance use, or certain assessments that require in-person observation. Therapists conduct risk assessments and may recommend in-person care when needed.
Emergency response: Therapists cannot physically intervene. Intake includes emergency planning, local crisis resources, and confirmation of the client’s location at each session.
Privacy and security
HIPAA and professional guidelines require use of secure platforms and reasonable safeguards. Therapists and clients should confirm the platform’s security and follow best practices (private location, use of headphones, device passcodes).
Informed consent should explain risks, limits of confidentiality, recordkeeping, and procedures for emergencies and technology failures.
Practical tips for clients
Choose a quiet, private room and use headphones.
Test your device and internet connection before the session.
Keep a list of local emergency numbers and a trusted contact available.
Be on time and treat the session like an in-person appointment (minimize distractions).
Talk with your therapist about what to expect and any concerns with telehealth.
Effectiveness Research shows telehealth therapy can be as effective as in-person therapy for many common mental health conditions, including depression, anxiety, PTSD, and couples therapy, particularly when delivered by trained clinicians and when clients have adequate technology and privacy.
If you’re considering telehealth therapy, check the therapist’s licensing, telehealth policies, platform security, insurance coverage, and emergency procedures to make sure it fits your needs.
