  1. I have a well balanced diet? 

  2. I consume fruits and vegetables everyday? 

  3. I drink at least 8-8 oz glasses of water a day? 

  4. I understand the appropriate quantity of food to consume ?

  5. I enjoy the type of exercise I engage in? 

  6. I watch my red meat intake and replace it with poultry, fish, and vegetarian proteins? 

  7. I am aware of my process food intake?

  8. I don’t smoke cigarettes ? 

  9. How much salt am I consuming? 

  10. I am limiting the amount of sugary drinks and food that I am consuming to 1 or 2 times a week? 

  11. I pay attention to my calorie intake and am not going over my recommended daily intake? 

  12. I exercise at least 30 minutes a day 3-5 times a week?

  13. I go to my regularly schedule doctors appointments and keep track of my health?
